Service Delivery Models

Evaluation and prioritization is required to determine the highest impact opportunities to pilot and test innovative new approaches to delivering services to individuals served by the Department. Services purchased in the community would also be a focus for innovation, including opportunities to integrate VA and non-VA service delivery. We are currently exploring 6 service delivery initiatives.

Models under this initiative would test the effectiveness of utilizing a team-based approach, while emphasizing prevention, health information technology, care coordination, and shared decision making among patients and their providers.

Models under this initiative would test the effectiveness of utilizing multi-disciplinary, integrated practice units organized around achieving improved outcomes for specific conditions or groups of conditions.

Models under this initiative would test the effectiveness of utilizing multi-disciplinary, integrated practice units organized around mitigating social risk and addressing the social needs of individuals in achieving improved outcomes and performance excellence.

Models under this initiative would test the effectiveness of utilizing a virtual-only approach to service delivery. To mitigate the risk and potential for adverse outcomes, individuals receiving services under the associated models would also have access to in-person care, should the need arise.

Models under this initiative would test the effectiveness of utilizing flexible and extendable service delivery architecture to improve performance excellence. Under this arrangement, individuals receiving services can either be transported to the most effective service delivery location or the necessary services would be delivered to where the individual is located.

Models under this initiative would test the effectiveness of co-locating all VA services (benefits, healthcare and memorial services) on one campus or in one building to improve performance excellence. Opportunities would also be explored to co-locate and integrate VA services in community locations.
